The Rowland Universal Dementia Assessment Scale (RUDAS), though employed in Indonesia, lacks demonstrable cross-cultural portability. The Indonesian context served as the backdrop for this study, which aimed to evaluate the reliability and validity of scores from the Rowland Universal Dementia Assessment Scale (RUDAS). With the involvement of 35 community-dwelling older adults, nine neurologists, and two geriatric nurses in the adaptation process, the Indonesian translation of the RUDAS (RUDAS-Ina) was completed by 135 older adults (52 males, 83 females) at a geriatric nursing center, aged 60 to 82. A consensus-building strategy was used to achieve face and content validity. Subsequent to conducting confirmatory factor analysis, a model with a single factor was revealed by the results. In research using the RUDAS-Ina, the reliability of the scores, as measured by Cronbach's alpha, was only marginally acceptable (0.61). The optimal management of 20mm non-functioning pancreatic neuroendocrine tumors (PanNETs) remains a subject of ongoing debate. The varying biological characteristics of these tumors complicate the decision-making process regarding resection versus observation.
A retrospective, multicenter cohort study of 78 patients who underwent resection of 20mm or smaller non-functioning pancreatic neuroendocrine tumors (PanNETs) at three tertiary care centers between 2004 and 2020 investigated the predictive value of preoperative radiological imaging and serological markers in determining the optimal surgical approach for these tumors. Enhancement computed tomography (CT) scans exhibited a non-hyper-attenuation pattern, characterized by hetero or hypo-attenuation, and implicated main pancreatic duct (MPD) involvement. Serological testing indicated elevated serum elastase 1 and plasma chromogranin A (CgA).
Small, non-functional PanNETs showed lymph node metastasis in 5 of 78 cases (6%), WHO grade II in 11 of 76 (14%), and microvascular invasion in 9 of 66 (14%). A noteworthy 20 of 78 (26%) cases displayed at least one of these adverse pathological signs. During preoperative evaluations, hetero/hypo-attenuation was observed in 25 of 69 patients (36%) and, separately, MPD involvement was seen in 8 patients (11%) of the 76 evaluated.
